Paper waivers get lost, and a photo of a signed page is not a record. Patron gives you a consent form you build yourself, that clients sign on their phone, and that saves as a signed PDF you can pull up years later.
Build the form your studio needs
Start from the standard health and consent questions, then add your own. Ask about allergies, medical history, placement, or anything your state requires. You decide what appears, and you can update it when the rules change.
Signed before the needle, on their phone
The client fills out the form and signs on their own device, so it is done and stored before they sit down. No clipboard, no printer, and no photo of a signature nobody can read later.
Every signature is kept, and easy to find
When someone signs, Patron freezes the exact version of the form they agreed to. If you change the form next month, old records still show what that client saw and signed. Each one exports as a clean PDF, and you can pull up any client's signed waiver later by searching, instead of digging through a drawer or a separate app.
Included, not billed per form
Some consent tools charge for every waiver, or for every extra field you add. In Patron, consent forms come with your plan. Collect as many as you need, keep them all, and never pay a fee per signature.
Age and readiness at a glance
The form captures date of birth, so the front desk can see who is under 18 and whether a waiver is signed before a client is checked in. The full answers stay with you and the artist, not on any public screen.

Do you charge per form or per signature?
No. Consent forms are part of your plan. You can collect and store as many as you need without a per-form or per-signature fee.
Can I use my own consent questions?
Yes. You build the form from a set of standard questions and add your own on top, so it matches what your studio and your state require. Update it whenever you need to.
How do clients sign?
On their own phone or device. They fill out the form and sign before the session, and the signed copy is stored right away. No paper and no printer.
What happens to a signed form if I change the form later?
Patron keeps a frozen copy of the exact version each client signed. Editing the form going forward does not change past records, and every signed form exports as a PDF.
Does the front desk see medical answers?
No. The front desk sees only what it needs to check someone in, such as whether a waiver is signed and whether the client is over 18. The full answers stay with you and the artist.
